Percutaneous Epididymal Sperm Aspiration & Testicular / Epididymal Sperm Aspiration

PESA

Percutaneous Epididymal Sperm Aspiration(PESA) is a non surgical process that invovles passing a small needle directly into the head of epididymis via scrotal skin

TESA

Testicular Sperm Aspiration (TESA) involves insertion of needle in the testis & aspirating fluid & tissue with a negative pressure.

PESA

PESA is a treatment method employed when infertility results because of the male partner. It is one of the sperm retrieval processes that is carried out in cases of Obstructive Azoospermia or lack of sperm in semen. It is an invasive technique that involves extracting the sperm form the Epididmysis.

Working

PESA

It is performed in a hospital set-up, under general or local anesthesia. Pre-testing might include blood tests and testicular biopsy. In this procedure, a fine needle is passed through the skin of the epididmysis to retrieve the sperm.

PESA is usually done on the same day as the eggs are collected from the female partner who is going through the IVF cycle. The sperm is retrieved through PESA and the inserted into the eggs or the cytoplasm of the eggs with the help of ICSI. PESA does not usually require hospitalization and the person can be discharged on the same day.

TESA

It is performed in a hospital set-up, under general or local anesthesia. Pre-testing might include blood tests and testicular biopsy. Unlike in PESA where a fine needle is passed through the skin of the epididmysis to retrieve the sperm, here, the needle is passed through the skin directly into the testes to collect a few small number of seminiferous tubules themselves. Once collected, the tubules are dissected in the laboratory to search if any content of the sperms are present. This is because sperms are produced in the testes in these tubes called the seminiferous tubules. TESA does not require hospitalization. The person can be discharged on the same day.

Advantages

PESA
  • Good success rate:When used in combination with IUI and IVF, PESA ensures high success rates of fertilization.
  • Simple procedure:Simple and short procedure of 20 minutes, with quick recovery. Requires few instruments and materials with no surgical exploration.
  • Better alternative for Vasectomy reversal surgery:PESA with IVF offers a better alternative than Vasectomy reversal surgery. Requires no major surgery, retention of contraceptive effectiveness, high pregnancy rates and less time to pregnancy.
TESA
  • High Pregnancy Rate:Better than other sperm extraction technique as it requires less amount of testicular tissue.
  • Safe procedure:Less vascular injury compared to other procedures and identification of blood vessels is easier. Also requires few instruments and no surgical exploration. Post-operative discomfort is minimal.
  • Affordable:It can be performed at a low cost and is a fast procedure. It is also a repeatable procedure.
